STUDENT ATHLETE ELIGIBILITY
 

 
 

 
*Sixth-graders are now eligible to participate in middle school athletics under the following State of Georgia policy:

Student-athlete eligibility consists of two elements — general athletic eligibility and academic eligibility. Students seeking to participate in athletics at an Atlanta Public School must meet both elements.

High School Eligibility
In order to meet general athletic eligibility, students that tryout for or participate on an Atlanta Public Schools high school athletic team must meet the following requirements:
1) Have a current Student Athletic Participation Form on file at his or her school (Students that participate in off-season conditioning must also have this form on file);
2) Have a pre-participation physical form on file;
3) Meet academic eligibility certification requirements (See Georgia High School Association (GHSA) Constitution and By-Laws);
4) Be an amateur (see GHSA guidelines for amateurism) in the sport he/she participates.

Middle School Eligibility
1) Have a current Student Athletic Participation Form on file at his or her school (Students that participate in off-season conditioning must also have this form on file);
2) Have a pre-participation physical form on file;
3) Meet age and academic requirements;
4) Pass 5 classes the previous semester.

Student Athletic Participation Form
The Student Athletic Participation Form consists of four parts:
• Parent Consent for Athletic Participation and Emergency Medical Treatment
• Insurance
• Student Media Release
• Physical Examination

Students may not participate in interscholastic athletics without the signed certification of a parent or guardian in all three parts.

Parent Consent for Athletic Participation and Emergency Medical Treatment
In this section of the Student Athletic Participation Form, the parent or guardian gives consent for the student to participate in athletics at a specified Atlanta Public School. Further, by signing this section, the parent or guardian certifies that they understand the risk to the student by participating in interscholastic athletics and consents to emergency medical treatment.

Insurance Options
Under no circumstances will a student participate in interscholastic athletics without medical insurance. The parent or guardian must certify that the student is covered by a group plan or Medicaid (Option No. 1) or that the parent or guardian chooses to use the limited excess benefit plan through Atlanta Public Schools (Option No. 2) (See Sports Medicine chapter).

Student Media Release
In this section parents or guardians consent to allowing the student to be recorded via photography, video, or voice recorder and that their image and likeness may be used by Atlanta Public Schools for promotional activities and teaching practices.

Pre-Participation Physical Evaluation Form
Every student that participates in any Atlanta Public Schools athletic program must have an annual physical examination. The examination must be given by a board certified medical doctor and recorded on the Atlanta Public Schools Permission to Participate and Physical Examination form. No other form is acceptable. This form consists of one section, which must be completed and signed by the student and parent or guardian (the history form) and the physician of record (the clearance form and the physical examination form).

Academic Eligibility for High School Students
Eligibility is a major concern for all athletic directors and coaches who each season must verify that each athlete has satisfied the academic standards for scholastic eligibility as adopted by the GHSA and the Atlanta Board of Education. Therefore coaches must be especially attentive to each student/athlete’s academic standing and assisting those with tutoring who are having difficulties in the classroom. Academic progress reports should be arranged for all student athletes before, during and after the season. It is necessary to communicate to administrators, teachers, and parents, the steps that are being taken to ensure the athletic eligibility of all student athletes.

In accordance with GHSA regulations, students regardless of whether they are participating in varsity, sub varsity or “B” team competition must be academically eligible in order to participate, practice and/or try out for interscholastic activities. In order to be considered academically eligible a student is required to pass (with a grade of 70 or higher) five (5) Carnegie unit subjects counting toward graduation, or their equivalent, the semester immediately preceding participation, with the exception of first semester ninth grade students.

Students must have passed at least five (5) courses the previous semester

12th

Students must have accumulated sixteen (16) Carnegie units the beginning of 4th year

Students must have passed at least five (5) courses the previous semester
